Documents Required

Mandatory Documents

Visa Application Form

• Fully completed and signed application form.

Photograph

• One recent passport-size photograph (not older than 6 months),

Passport

• Original passport and copy of all relevant pages.
• Issued within the last 10 years.
• Valid for at least 3 months beyond the intended return date.
• At least two blank pages required.

UK Residency

• Provide a UK eVisa printout from UKVI with a valid share code.
• Must be valid for at least one month beyond the date of exit from the Schengen area.

Medical Documents

• Certificate from a licensed medical practitioner in the UK confirming the need for treatment in Switzerland.
• Official letter from the Swiss hospital or clinic confirming:
  • Acceptance of the patient for the required treatment.
  • Dates of admission or consultation.
  • Confirmation of treatment costs and pre-payment (if applicable).
• Proof of accommodation in Switzerland during the entire treatment period:
  • Hospital stay confirmation, or
  • Hotel/residence booking or lease agreement if outpatient care is planned.

Proof of Finance

• Recent bank account statements (dated within 7 days of application) covering the last 3 months, showing the applicant’s name and sufficient funds to support the stay and treatment.
• If treatment is financially sponsored by another individual:
  • Sponsor’s bank statements (last 3 months)
  • Sponsor’s employment letter
  • A signed declaration of financial responsibility
  • Copy of sponsor’s passport
  • Proof of relationship (e.g. birth/marriage certificate)

Proof of Travel

• Confirmed return flight reservation or onward travel from Switzerland.
• If travel between multiple Schengen countries is involved, full itinerary and accommodation must be provided.

Travel Insurance

• Must be issued by a UK or Schengen-based company.
• Must include:
  • Applicant’s name and date of birth
  • Full Schengen coverage
  • Validity for the entire duration of stay
  • Minimum coverage of €30,000 for emergency medical expenses and repatriation
  • Must be printed on official letterhead with provider contact details
  • If covered under a bank scheme, provide a dated confirmation letter (within 30 days) with policy details

For Minors

• Signed visa application form by both parents or legal guardians
• Birth certificate
• Letter of consent from both parents (if not travelling together), with copies of their passports
• For sole custody cases, appropriate legal documents such as a custody ruling or death certificate must be submitted
• Translation into English, German, French, or Italian if not originally issued in one of these languages

Photo Specifications

• Must be recent (within 6 months)
• Size: 35–40 mm wide with face covering 70–80% of the image
• Sharp, clear, and in focus, with no marks, creases, or blurring
• Printed on high-quality photo paper (digital prints accepted if high resolution)

Style and Lighting

• Neutral expression, mouth closed, eyes open, and looking directly at the camera
• Taken against a plain, light-coloured background with no shadows or reflections
• Natural skin tones with balanced brightness and contrast

Glasses and Head Coverings

• Glasses: no tinted lenses or flash reflections; frames must not cover eyes
• Head coverings allowed only for religious reasons, but the full face must be visible

If the submitted photograph does not meet these specifications, the application will be deemed incomplete and the applicant will be required to provide a compliant photo before the application can proceed.
